(Only QUALIFIED Healthcare Professionals accepted) Family-NP - Position Overview: To provide primary care services to ACMC Long-term (LTC) and Skilled Nursing (SN) patients focusing on disease prevention and|or stabilization, and management of chronic and episodic illness in collaboration with the programs Physician providers in various facilities throughout Ashtabula County, including ACMC Skilled Nursing Unit. Works with the interdisciplinary team to establish measurable goals and to develop and manage individualized plans of care for LTC and SN patients.
Monday thru Friday No Call Will Work With One|Two Internal Medicine Physicians $85,000 - $95,000 Salary Range Depending on Years of Experience Car Mileage Reimbursement Based on IRA Guidelines
Performs comprehensive assessments of patients for the purpose of developing complete lists of medical diagnoses and plans of care, in collaboration with their Nursing Home physician Evaluates patients with new symptoms or episodic illnesses and orders appropriate treatments and interventions Performs routine monitoring of chronic illnesses Performs annual nursing home reviews Performs those interventions identified in the Interdisciplinary care plan as appropriate for the PCP and|or NP|PA Patients in Interdisciplinary team meetings Visits and provides assessments and care for patients in Skilled Nursing Facilities, Assisted Living, and the ACMC Skilled Nursing Unit. Visits patients in acute care settings as appropriate to help with treatment decisions and discharge planning Documents all care and services in succinct and accurate ways consistent with professional standards in the appropriate electronic health record Establishes and maintains effective working relationships with patients, family members, caregivers, medical staff, and nursing home staff Participates in provider Chart reviews as requested by the Medical Director(s) or the Director of Advance Practice Providers Recognizes, identifies, and implements opportunities to help meet Utilization goals Supports the fundamental mission of the ACMC Long-term Care and Skilled Nursing program Participates in quality assessment and performance improvement projects and workgroups Travel to facilities within Ashtabula County required
Education: Bachelors of Science in Nursing, Masters Degree in Nursing and a graduate of an accredited Nurse Practitioner program.
License|Certification: Licensed by the Ohio Board of Nursing in Nursing and as a Nurse Practitioner
Experience: One or more years of Nurse Practitioner experience in either and office setting focused on Family Practice, Internal Medicine, or Geriatrics, Long-term care setting preferred
